Community Health Events

Saving Strokes: Rehabilitation Through Golf (FREE)
California Pacific Medical Center (CPMC) is partnering with the American Stroke Association for San Francisco’s fifth annual Saving Strokes, an event that’s all about stroke rehabilitation through golf. Stroke survivors don’t need to be golfers to join this fitness workshop. Each participant begins the program with a blood pressure check, followed by a check at the physical assessment station, where a physical therapist evaluates range of motion. Following the evaluation, participants have individual appointments with an experienced golfer who provides 30 minutes of instruction. Lunch is provided at noon.

Total Hip Replacement: Preoperative Patient Education Program (FREE)
This preoperative program provides patients with information about hip surgery. Topics include anatomy of the hip joint, prosthesis equipment required after surgery, self-care management, using a walker, preoperative exercises, modifications in the home, preparing for surgery, length of stay in the hospital, pain management and preventing hip dislocation. Patients and their family and friends are encouraged to attend.

Healthy Heart Exercise
This supervised exercise program is for people with high blood pressure, high cholesterol or diabetes, as well as those who are obese or sedentary.

Nutrition and Weight Management

BodyLOVE
Is yo-yo dieting making you overweight? Are you tired of feeling out of control with eating? Join us for BodyLOVE, a 12-week program with the nutritionist, where you’ll learn the physiology of weight loss, optimal eating and how to manage cravings; discover how to use yoga, movement and guided imagery to reduce stress-related overeating; and take part in a women’s discussion group led by therapists specializing in eating and body image concerns.

Women who are diagnosed with cancer receive free cosmetics, tips on makeup and hair styling, plus a free wig and scarves.

These programs assist women in cancer treatment and recovery. Services include

Diabetes and Self-Mangement

CPMC Center for Diabetes Services
This American Diabetes Association (ADA)–recognized program provides classes and individual counseling sessions for adults and children with type 1 or type 2 diabetes. The center also offers prevention classes for those at risk for diabetes plus support groups and counseling. Staff speaks English, Tagalog, Spanish and Chinese.

Diabetes — Individual Education (FREE)
Individual appointments for diabetes education. Available for those needing specific counseling (for example, insulin therapy initiation) or for those with special needs (physical limitations, non-English speaking, etc.). The American Diabetes Association recognizes this education service as meeting the National Standards for Diabetes Self-Management Education. This program is patient-focused and taught by nurses, dietitians and a clinical social worker. Referral required. Call for appointment.

Diabetes — Type 2 Education: Four-Class Series (FREE)
A series of four classes for people with type 2 diabetes. The American Diabetes Association recognizes this education service as meeting the National Standards for Diabetes Self-Management Education. The series of classes and one-on-one appointments are tailored to meet individual needs. This dynamic, research-based program is patient-focused and taught by our expert staff of nurses, dietitians and a clinical social worker. Class 1 held most Mondays or Fridays, either 9 a.m.–noon or 1:30–4:30 p.m. Call for dates and times. Diabetes — Continuous Glucose Monitoring (FREE)
An advanced program that allows for real-time, continuous glucose readings. This information provides in-depth exploration of challenging glucose patterns to optimize control. Ideal for those taking insulin (both type 1 and 2), those with complicated diabetes medication regimens or individuals who experience frequent episodes of hypoglycemia (low blood glucose). During the first session, participants attend a class and are placed on the continuous glucose monitoring device (Dexcom or Guardian). One week later, the device is removed, data is downloaded, and the information is analyzed. Patients new to our center will need a 1½-hour, pre-class appointment.

Diabetes — Intensive Management Series: Two-Class Series (FREE)
An advanced insulin management program designed for people with type 1 diabetes and people who are interested in an insulin pump. Patients new to our center will need a 1½-hour, pre-class appointment. Topics include carbohydrate counting, finding your insulin-to-carbohydrate ratio and adjustment of insulin to meet your target goal.

ADA-recognized, the Diabetes Center at CPMC, St Luke’s Campus, offers a multidisciplinary, caring, nurturing, bilingual team approach for the care of people with diabetes. Our nurses and dietitians coordinate care with your physician to ensure that you are educated about diet and exercise, blood sugar monitoring, and medications. Individual consultations are available. California Smokers’ Helpline
The California Smokers’ Helpline is a free telephone program that can help you quit smoking. Funded by the California Department of Health Services, the hotline provides self-help materials, referrals and one-on-one counseling over the phone. Anyone in California can call, whether they are currently smoking, have already quit or want information for a friend or relative.
